GENERAL REQUIREMENTS
Payment receipt for the visa solicitation, from the Union colombian Bank(Banco Unión Colombiano). The value is equivalent pesos, in cash, US$65 for every request. This receipt is non-transferable and it will have validity of one year.
The payment must be cancelled in the office located in the "carrera" 47 #22-99, Eliana Building, or in the "Carrera" 8 #14–45 (Jimenez avenue). The desprendible payment is a bar code that sticks to your passport. Do not remove it until the consul section has processed your visa, unless you travel another country. In this case keep it in a secure place.
Current passport and the previous ones. Bear in mind that the current passport must have a minimum of 6 months of validity to enter the United States.
Identicifation. Citizenship card for those who are older than 18 years. Idetification card for those who are younger than 18 years of age. Those who have just turned 18 years must present his or her Citizenship card or receipt of certification from the government Registry and the corresponding note in the passport.
Recent photo, in color, size 3x4, sticked to the 2 page of the form. Black and white photos are not accepted.
If you have children, you must submit a copy of his or her birth certificate.
I-20A form (fill out only the neccesary pages) or I-20M form, received from a University or School in the United States.
Personal financial information. (information of the parents, if one depends on them) that shows sufficient economical stability to cover all the expenses of the studies, or letter concerning a scholarship or grant.
Proof of having a usable medical insurance in the United States.
